Using Detailed Comparative Data and Predictive Analytics to Improve Population Health and Optimize Resource Use for Value-Based Payment

Improving population health requires a broad analytic perspective, with a dual focus: (1) identifying potential “outliers” for timely intervention, and (2) redesigning mainstream care processes for typical patients, to reduce variation and achieve optimal outcomes while minimizing overall cost. This presentation will highlight three ways in which medical groups are using Humedica MinedShare and AMGA’s Anceta Collaborative Data Warehouse to understand and improve population health. The groups  will share how they are using MinedShare in case management to improve ongoing care for patients and also optimize payment under risk-based programs such as Medicare Advantage and ACO pilots with commercial payers. The presentation will highlight how groups and others are using the Anceta collaboratives for shared learning about the practical use of these tools to drive improvement.

Upon completion of this activity, participants will be able to explain how Anceta and Humedica enable AMGA members to under­stand variation in practice and intervene to improve quality and reduce cost. They will also be able to discuss using comparative data for evaluating clinical performance and predictive analytics for identifying high-risk patients
